What makes it reset itself? I know the description "once a mission" is wrong.

The rez signet itself can only be used once per mission. You may be thinking of the monk resurrect spells that can be used multiple times. The rez signet resets when you return to an outpost or town.

I believe it also resets on cutscenes and whenever you kill a boss.

It Resets after every Morale Boost.

Just a note, all of your skills will recharge once you kill a boss - including Rez Signet.
