How/When do you leave the bloody island. I want to get to the real meat of the content.

Follow all the quests in the main line. You'll eventually get out. BTW don't be in a big rush. You end up in the Marketplace and you'll be up against very high level foes compared to how it was in Prophecies. In Prophecies, you could leave pre-Searing at level 7 and do fine. Not so in Factions. You want to be more like 12 at least.

Once you leave Seitung Island you will be against groups of level 20 mobs.

Is there anything not quest related that you can't get once you leave? Or is it possible to go back? I am under the impression you can go back based on the preorder event day because people brought their tyrian characters to that area.

You can return at any time to the intial island. As for what you have to do:
*spoilers*
Keep following the primary quests, after you complete the second mission you can leave the island and reach the mainland. At which point you are up against level 20 creatures. At this time you can run north up through the "byway" and reach the City Center. From here you can get your 1500 gold max stat armor, buy any of the regular skills for your character, and travel to Tyria if you so choose.
