When a mission is in a town a sheild is there.... is it always completed when two swords are cross it. Or are there some with only one sword.
In Prophecies, all missions have 2 swords (mission and bonus).
It's different in the other chapters. Factions and Nightfall have 3 swords for all missions (normal, expert and master rating).
Eye of the North's missions have no bonusses or extras.
Technically, EotN doesn't have Missions. They are Primary Quests.
Obviously the main difference is that a party wipe doesn't mean a failure and return to the last outpost.
Bonus objectives are not necessary to finish the mission, though. You can do only the mission itself without completing the bonus objective and you'll be able to continue the game.
In Factions and Nightfall, you only need one sword for the mission to be considered completed. The Expert and Master rewards simply mean that you reap more experience and more gold.
Most missions in Factions are time-based... Complete it under x, y or z amount of time and you'll get Standard, Expert or Master reward.
Most missions in Nightfall have secondary objectives... You can either not do those at all (Standard reward), do a few of them (Expert) or do them all (Master).
In both campaigns there are a few missions where it's not time-based in Factions or where it IS time-based in Nightfall. Those are a minority though.
For most missions in Prophecies you'll get one sword as soon as you've completed the bonus objective. So it is possible to have one sword even if you haven't finished the mission for whatever reason (lack of time, failed).

Aside for bonus gold & XP, getting the bonus or master's reward also contributes to the Protector (NM) or Guardian (HM) title tracks.
